all plans allow Hardship Distributions and/or Participant Loans. Also,
certain plan provisions and Government regulations must be met in order
to qualify for a Distribution, Hardship, or Loan. Simply completing and
remitting a form does not entail that your Distribution, Loan, or Hardship
request is automatically approved. All requests must receive approval
from the Plan's Trustee or from the Company sponsoring the Qualified Retirement
